Granite VNA, is a not-for-profit licensed and certified home health and hospice care provider that has served New Hampshire since 1899. The agency serves people of all ages in 72 communities across the Greater Capital Region and Central New Hampshire by providing home care, hospice care, pediatric and maternal child health services, and wellness programming. Home Care 

• Assist with medication changes and management
• Manage conditions such as heart failure, COPD, and diabetes

• Plan post-surgical needs such as mobility, activities of daily living and incision site care
• Specialized therapy for lymphedema, vestibular, joint replacement, and other orthopedics
