The Visiting Nurse Association of Franklin, also known as Franklin VNA & Hospice, is a not-for-profit licensed and certified home health and hospice care provider that has served residents of all ages in 14 communities in Merrimack & Belknap Counties since 1945. Our agency provides home care, hospice care, personal home services, and community health clinics. Our services promote dignity, compassion, independence, and help maximize the choices available to residents, the majority who are elderly or disabled. Services and programs we provide are:

Home Care


  • Skilled nursing to assist with medication changes and management, IV therapy, wound care management, pain management, blood drawing, along with education and management of conditions such as heart failure, COPD, and diabetes.

  • Rehabilitation services that include Physical, Occupational, and Speech Therapy.

  • Medical Social Services for support and resource counseling for management of social problems related to illness and medical treatment.

Hospice Care

  • End of life care provided by compassionate and professional staff and trained volunteers to support the patient, family, and caregivers in the home. Support services include nursing for pain and symptom management, education. Volunteers for respite care, companionship, and comfort. Spiritual and emotional support, bereavement, and grief services. Along with on-call support available 24 hours a day. Medical equipment, other health supplies, and therapy services are included in hospice services.

Personal Home Services


  • Personal Care to assist with bathing, dressing, and feeding. 

  • Homemaking that includes light housekeeping, grocery shopping, laundry assistance, light meal preparation, errands for necessities, letter-writing, and providing companionship.

Community Health Services


  • Community Health Clinics that include blood pressure monitoring, foot care, flu immunizations, and education trainings. 

  • Grief Support Groups

As a non-profit organization we are invested in ensuring that patients receive services that will allow them to stay safely at home regardless of their insurance status or their ability to pay. Your support enables us to:


  • To provide services to patients without insurance, or where there are gaps in insurance coverage.

  • Provide wellness programs.

  • Provide Grief Support Groups and bereavement programs.

  • Provide Community Health clinics and educational trainings to the communities we service.

  • Provide training and educational opportunities to employees.
